The purpose of this role is: - To provide advanced triage and physiotherapy services as part of the SlainteCare Programme (https://assets.gov.ie/22607/31c6f981a4b847219d3d6615fc3e4163.pdf) , to support service redesign initiatives aimed at delivering the safe transition of patient care between Acute & Community/Primary Care services, to facilitate the delivery of integrated care for patients with MSK conditions, and to ensure that patients receive appropriate care at the earliest possible time, as close to home as possible. - To contribute to the mainstreaming of the improved musculoskeletal pathways of care between Community/Primary Care and Acute services, through the establishment of multidisciplinary outreach MSK interface clinics. - To work with orthopaedic and rheumatology consultants and GPs undertaking triage of musculoskeletal referrals to improve the pathway of care between Community/Primary Care and Acute services, thereby facilitating efficient access to orthopaedics, rheumatology, physiotherapy or other service as required.

The purpose of this Clinical Nurse Specialist (Smoking Cessation)  post is to: Deliver care in line with the five core concepts of the role set out in the Framework for the Establishment of Clinical Nurse Specialist Posts, 4th edition, National Council for the Professional Development of Nursing and Midwifery (NCNM) 2008. The Clinical Nurse Specialist Smoking Cessation will deliver a specialist cessation service in line with the National Standard for Tobacco Cessation Support Programme.  Clients who are patients in Cavan and Monaghan Hospital may be referred by a health professional or through self- referral. Clients will be registered for a tobacco cessation support programme.  The purpose of the programme is to enable the client to quit and stay quit and to assist the client in coping with cravings and withdrawal symptoms. Support programmes are client centred and provide behavioural support sensitive to the individual’s preferences needs and circumstances. (National Standard for Tobacco Cessation Support programme, March 2013). Smoking Cessation Support programmes will be delivered on an individual/one to one  or group basis, in both in-patient and out-patient settings.  Phone support will also be provided where appropriate.   Details of Service/Background to the post: Tobacco use is the leading cause of preventable death in Ireland.  Each year at least 5,200 people die from diseases caused by tobacco use. This represents approximately 19% of all deaths. 1 in every 2 smokers will die of a tobacco related disease and on average smokers lose 10 quality years of life.  Most smokers say they want to quit and about 40% of all smokers try to quit every year. Only a small fraction (2–3%) actually successfully quit each year. Smokers can double their chances of quitting using behavioural support and/or pharmacotherapy. (TobaccoFree Ireland, Report of the Tobacco Policy Review Group, Department of Health 2013). A Cochrane Review published in 2013 found that advice and support from nursing staff could increase people’s success in quitting smoking, especially in a hospital setting and by nurses whose main role is smoking cessation. (http://www.cochrane.org/CD001188/TOBACCO_does-support-and-intervention-from-nurses-help-people-to-stop-smoking) The strategy document Healthy Ireland A Framework for Improved Health and Well-Being 2013 – 2025 includes as a key area for action, a reduction in smoking prevalence.  The HSE has adopted a health service wide Tobacco Control Framework (February 2010) to address this single greatest cause of ill health and mortality in the Irish population. Tobacco cessation services are recognised as an important element of a comprehensive tobacco control strategy. A National Standard for Tobacco Support Programme (March 2013) has been developed which sets out to define and describe the HSE’s National Standard for Tobacco Cessation Support Programmes delivered by cessation specialists with dedicated time to deliver specialist support.   Location of Post: Cavan and Monaghan Hospital. There is currently one permanent, whole-time post available in this location. The position of Clinical Nurse Manager II (CNM II) Clinical Facilitator will initially be based in the Emergency Department at Cavan & Monaghan Hospital on a temporary/specific purpose basis.  The post holder will then revert to the Practice Development Department when the substantive post holder returns. The position has a pivotal role in: - Enhancing outcomes for patients through assessment of standards of nursing, midwifery and healthcare assistant practice; the planning and implementation of appropriate education and professional development and the ongoing audit and evaluation of standards and patient outcomes. - Providing a quality educational and supportive service to nursing staff/students on postgraduate training programmes. - Co-ordinating and managing an up to date educational/clinical programmes for relevant staff. The Nursing Practice Development Department consists of a team of senior and experienced nursing staff including the Nurse Practice Development Coordinator (Assistant Director of Nursing, HoD), Clinical  Placement Coordinators (x 2), Clinical Nurse Facilitators, Resuscitation Training Officer CNM2, Paediatric Practice Development Facilitator. The Practice Development (PD) team supports nursing services and the multidisciplinary team across both Cavan General Hospital and Monaghan Hospital in implementing evidenced based practice to promote safe and effective patient care. The PD team also supports the clinical learning environment for undergraduate and postgraduate clinical placements.   The CNM II Clinical Facilitator Emergency Department will take a lead in supporting nursing staff, in delivering safe and effective nursing care, which is evidenced based and patient centred. Support and guide nursing staff undertaking courses relevant to the Emergency Department such as the Higher Diplomas in Emergency Paediatrics or Adult Nursing. The postholder will be working within all the clinical areas of the Emergency Department i.e. AMAU, minor injuries unit, ED isolation stream and will be supporting the development of nursing care for all patients presenting to the Emergency Department.   The CNM II Clinical Facilitator Emergency Department post will assist and support the Nurse Practice Development Co-Ordinator, in a systematic approach, to assist nursing teams to examine their practice and identify how it can be improved, with a person-centred focus. This will be done through staff development, continuing education programmes, quality improvement initiatives, development of clinical expertise, effective communication, managerial and leadership skills.

This is a full-time appointment on a Type B basis under the Consultants’ Contract 2008 by the Health Service Executive to be based at Connolly Hospital 39 hours per week.   The Catchment Area extends across West Dublin, Meath and Kildare, where much of the recent housing development has occurred to support the rapid growth of Greater Dublin and its commuter belt hinterland. The Catchment area population has grown dramatically over the past 15 years and currently stands at approximately 350,000. Most of this is urban-based but there is a significant rural sector, all dependent on Connolly Hospital for their acute Hospital needs. The role includes patient assessment, supportive therapies, patient/family education, counselling and support and interdisciplinary and multidisciplinary communication including strong liaison with Beaumont Hospital. The CNM2 role is a critical one in the advancement of quality care delivery through audit, research and knowledge sharing at local and national levels. One of the main responsibilities for the CNM2 will be in the management of the psychological, emotional, informational and clinical needs of patients with colorectal malignancy and their significant others. The CNM2 role is also crucial to the forging of strong links between the patient and the care providers.   Caseload CNM2 (Colorectal and Rectal Cancer) will focus initially on patients with malignant diseases; Colon, Rectal and Anal Cancers attending Our Lady of Lourdes Hospital in line with the Scope of Practice of CNM2. Every year there are patients accessing Beaumont hospital for rectal cancer surgery who were managed for components of their care and treatment, in Our Lady of Lourdes Surgical Department.
