The Senior Medical Social Worker will work as part of both the Medical Social Work team and the Sycamore MDT in Connolly Hospital. This role involves the facilitation and provision of emotional support and counselling to patients and families in their critical care journey.   The completion of psychosocial assessments to ensure there are early identifiers to barriers to care and recovery for the patient will be required so as to thereafter enable patients to reach their potential post critical care.   Collaborative multidisciplinary working and autonomous practices regarding caseload management and priorities are key requirements for this position.   RCSI Hospitals Group covers a population of over 800,000 people.  Connolly Hospital’s Catchment Population of over 331,000 covers Dublin West, North Kildare and South Meath.   Connolly Hospital Blanchardstown is a major teaching hospital providing a range of acute medical and surgical services, day care, outpatient, diagnostic and support services.  Emergency services are provided on a 365-day, 24 hour basis.  Multi-disciplinary teams representative of medical, nursing, allied health professionals, management and general support staff play a pivotal role in the development, delivery, monitoring and evaluation of these services.  The hospital has a current bed complement of 391 beds.   The hospital is affiliated to the Royal College of Surgeons in Ireland (RCSI), Dublin City University (DCU), University College Dublin (UCD), Trinity College Dublin (TCD) and the Institute of Technology for teaching a variety of undergraduate students.   The person appointed to this post will also be responsible for providing supervision to medical social workers and students as required.

The post holder will provide physiotherapy input to ensure delivery of an effective and efficient multi-disciplinary frailty Intervention team (FIT). FIT team aims to provide the following:   - Rapid access to MDT assessment in the Emergency Department - Complete comprehensive geriatric assessment - Appropriate streaming of patients to internal or external rehabilitation pathways including community integrated care services (ICPOP) - Avoid unnecessary hospital admissions while promoting timely, safe and appropriate discharge of patients.   To work in conjunction with other team members in co-ordinating and developing the service to meet the needs of the population it serves in line with the objectives of the organisation. To complement existing older persons services to provide highest quality physiotherapy care to frail older persons presenting to Connolly Hospital. Collaborate with the Emergency medicine, acute medicine and older persons (COTOP) services.   To be responsible for the provision of a high quality Physiotherapy service in accordance with standards of professional practice. To work with Physiotherapy Manager in ensuring the co-ordination, development and delivery of a quality, client centred physiotherapy service across and between networks in the geographical area. Flexibility is required as the post holder may be required to work across the Stroke & Older Person Rehabilitation service based on service needs.   The person appointed to this post will work as part of a multi-disciplinary team delivering a co-ordinated approach to client care. This includes research and prior experience in development work on topics and techniques related to Older Persons.   Be responsible for the provision of a high-quality Occupational Therapy input to the Acute Older Person Rehabilitation Service and will carry out clinical and educational duties as required. The role of the CNSp. CVD was developed to: provide expertise and specialist nursing services to patients with a cardiovascular condition both in the ward, hospital outpatient settings and in primary care. The role of the CNSp CVD will differ according to the needs and configuration of established cardiovascular services at each site.   The post holder will act as a liaison between acute cardiology services and integrated cardiology services in the community and other agencies, to deliver effective evidenced based care, using resources efficiently to achieve the best possible outcomes for patients with cardiovascular disease in keeping with agreed models of care and HIQA standards.   The post holder will work with colleagues across the acute cardiology services and integrated care services to develop and implement ambulatory care pathways for cardiovascular disease and associated co-morbidities, to improve the transition of patients between primary and secondary care.  The post holder will work as part of a MDT and in close liaison with their associated Consultant Cardiologist to deliver coordinated evidence based care for patients.   The CNSp CVD will support the implementation of a model of integrated care which is focused on enhancing the management of care for patients between Primary (General Practice) and Secondary (Hospital) Care, thus optimising the patient’s quality of life and contributing to an integrated health service. The role supports the implementation of Sláintecare through the provision of community diagnostics and shifting treatment from the acute sector to the community.   The primary focus of the post holder will be to ensure that patients with cardiovascular disease receive timely and appropriate care through assessment, planning, implementation and evaluation of care delivery.   The person appointed to this post will work in the overall integrated cardiology service. This post is 50% acute based (Connolly Hospital) and 50% community based to work with the Integrated Care Consultant Cardiologist to develop integrated care services. Please note a portion of the appointees work will be carried out “offsite”. This means that the appointee will travel to the hubs to perform duties related to the role.     The CNSp. If deemed appropriate, site rotation should occur within the first 3-6 months of taking up the post. The Physiotherapists are actively involved in the Virtual Fracture Clinic (vFAC) initiative.  The Orthopaedic Physiotherapists are also embedded in the fracture clinics facilitating more efficient patient care.  Rehabilitation classes are operated by the Out-Patient Orthopaedic Physiotherapy service; providing an efficient and effective management approach to waiting lists.   The post holder will be based primarily in Out-Patient Orthopaedics, however flexibility is required as the post holder may be required to work on occasions, in Orthopaedic In-Patients.  Post holder will work primarily with acute Orthopaedic Out-Patients but will also have input into the management of the musculoskeletal caseload as required.  Currently there are six Orthopaedic Consultant Surgeons providing the Orthopaedic service in Connolly Hospital.   Professional reporting relationship for clinical governance and clinical supervision will be to the Head of Discipline through the professional line management structure.

The MSK Clinical Specialist Physiotherapist will work as part of the Physiotherapy Department, while also working closely with the Orthopaedic Consultant Surgeons & Consultant Rheumatologists in their respective clinics.  The person appointed will be required to: - Triage waiting lists and select patient for correct route to Physiotherapy, Orthopaedic surgery/management, Rheumatology Consultant management or other services. - Attend and participate in orthopaedic/rheumatology clinics on site and regionally as covered by consultant services. - Waiting list validation will be undertaken to remove patients who no longer require the service and to check for any duplication between Orthopaedics and Rheumatology - Patients will be sent appointments for clinics - Patients will be triaged in standalone and jointly held clinics by the Physiotherapist Clinical Specialist (Musculoskeletal Triage) and given follow up referral as required to Orthopaedic Consultant, Rheumatology Consultant, physiotherapy service /or other services - Provide physiotherapy treatment to more complex patients. - Improve the Clinical Pathway for Orthopaedics and Physiotherapy in the designated region. - Develop and implement a model of care which will support the HSE Transformation model for Primary and Secondary Care through the development of Interface Clinics. - Work with Orthopaedic Consultants/Registrars and Physiotherapists in outpatient clinics as part of the multidisciplinary team. This team may involve other disciplines involved in those teams’ e.g. nursing, occupational therapy and podiatry services. - The area covered will be as for the regional remit of the designated orthopaedic or rheumatology service within each HSE region and will involve outreach clinics. The Post holder will provide advanced triage and physiotherapy services as part of the SlainteCare Programme (https://assets.gov.ie/22607/31c6f981a4b847219d3d6615fc3e4163.pdf) , to support service redesign initiatives aimed at delivering the safe transition of patient care between Acute & Community/Primary Care services, to facilitate the delivery of integrated care for patients with MSK conditions, and to ensure that patients receive appropriate care at the earliest possible time, as close to home as possible. The Clinical Specialist Physiotherapist will contribute to the mainstreaming of the improved musculoskeletal pathways of care between Community/Primary Care and Acute services, through the establishment of multidisciplinary outreach MSK interface clinics. He/She will work with orthopaedic and rheumatology consultants and GPs undertaking triage of musculoskeletal referrals to improve the pathway of care between Community/Primary Care and Acute services, thereby facilitating efficient access to orthopaedics, rheumatology, physiotherapy or other service as required.   The “Care Redesign Fund” was made available as part of the National Service Plan 2020 to support the delivery of reform initiatives, identified through the Capacity and Access Joint Action Programme, that aim to move scheduled care closer to home, being more responsive to the needs and wishes of patients. It has been estimated that up to 60% of Orthopaedic and Rheumatology referrals could have been managed through an MSK physiotherapist service. This funding aims to enhance the existing MSK service in order meet this level of demand in the Acute setting and to develop Integrated Musculoskeletal (MSK) services through the establishment of community/primary care-based Musculoskeletal Interface clinics nationally.  This long-term aim will be achieved through service expansion and reconfiguration of the existing MSK services to develop an integrated pathway, predominantly based in the community/primary care setting   These clinics will be an extension of the existing MSK Triage Service, in the form of Outreach MSK Interface Clinics, and will also support physiotherapy services in Community/Primary Care for patients with MSK conditions. The Outreach MSK Interface Clinics will be the interface between Acute and Community/Primary Care service providers. The Clinics will comprise Clinical Specialist Physiotherapists, Orthopaedic and Rheumatology Consultants, and General Practitioners.  These clinics will undertake paper, Virtual and Face to Face triage of referrals to ensure patients are assigned to the optimum pathway of care for their referral. The Clinical Specialist Physiotherapists will work as a first contact practitioner at an advanced level in physiotherapy practice in line with competency level and scope of practice.   This model has proven effective in other countries for delivering safe, cost-effective alternatives and can be achieved in the Irish setting through outreach MSK Interface clinics, which will embed Consultant Governance and patient centred care.   The Physiotherapist Clinical Specialist (Musculoskeletal Triage) reporting relationship and accountability for the discharge of his/her contract is to: - Acute Physiotherapy Manager for physiotherapy clinical governance, attendance and service performance management. - Orthopaedic/Rheumatology Consultant for clinical governance in triage, advanced practice remit and service performance management.
