The CNS Integrated Care role supports the implementation of Sláintecare through the provision of community based ambulatory care by specialist teams in Ambulatory Care  hubs and in moving care and treatment from the acute sector to the community.   The Clinical Nurse Specialist (Diabetes) will develop effective and multi-disciplinary professional working relationships across all clinical groups and organisational boundaries promoting the Integrated Chronic Disease Model of Care and care of adult patients with Diabetes.   The post holder will liaise with the integrated diabetes services in the community ambulatory hubs and other HSE and associated health care agencies, to deliver effective evidenced based care, using resources efficiently to achieve the best possible outcomes for adults with diabetes in keeping with agreed models of care and HIQA standards.   The post holder will work with colleagues across the acute and community integrated care Diabetes services to develop and implement ambulatory care pathways for diabetes and associated co-morbidities, to improve the transition of patients between primary and secondary care.    The Clinical Nurse Specialist (Diabetes- Integrated Care)will: - Work within the Scope of Nursing and Midwifery Practice Framework (NMBI, 2015) and - Deliver care in line with the five core concepts of the role set out in the Framework for the Establishment of Clinical Nurse Specialist Posts, 4th edition, National Council for the Professional Development of Nursing and Midwifery (NCNM) 2008. - Support the multidisciplinary team who, in collaboration with the Director Of Nursing, Hospital Managers, Operations Leads, and Network Managers will be responsible for implementing and evaluating the delivery of the Model of Integrated Care for Patients within the acute hospital and within the community healthcare network and community healthcare organisation. - In line with the Model of Care, the CNS (Diabetes) Integrated Care role will involve working with the Consultant Endocrinologist in Secondary Care in-patient and OPD settings and in the Specialist Ambulatory Care Hub or where service needs require. The role of the CNS will differ according to the needs and configuration of established respiratory services at each site. The purpose of this Clinical Nurse Specialist - Respiratory Integrated Care post is to: provide expertise and specialist nursing services to patients with a respiratory condition both in the hospital outpatient settings and in primary care. The post holder will liaise between acute respiratory services and integrated respiratory services in the community along with other agencies to deliver effective evidenced based care.  They will use resources efficiently to achieve the best possible outcomes in keeping with the NCP Programme model of care and HIQA standards.    The person appointed to this post will work in newly formed Respiratory Integrated Care services.  The post holder will work as part of a multidisciplinary team delivering coordinated evidence based care for patients in primary care whilst liaising closely with secondary care. The CNS RIC will deliver nurse-led clinics to provide support to patients and their GPs in creating management plans, assessing inhaler treatments, assisting with diagnosis development and will provide education to patients and staff.  This post is 50% acute based and 50% community based. There will be a strong focus on service integration and team-working. This post will also involve the core elements of the CNS post to include clinical audit and research.   In order to ensure continuity of service to patients the CNS may be required to rotate/cover other parts of the integrated respiratory service such as Pulmonary Rehabilitation and Outreach.  This will also promote service integration and enhance skillset development.  It will be dependent on identified service needs at each site. If deemed appropriate, site rotation should occur within the first 3-6 months of taking up the post. The Clinical Nurse Specialist (RIC) caseload will focus initially on the following patient groups. - COPD - Asthma    Location of post This is an appointment to Cavan and Monaghan Hospital by the Health Service Executive. The commitment for this post will be for 37.5 hours per week to work within the associated community-based ambulatory care hub. This post may be subject to restructuring in the future to facilitate the re-organisation of services in line with new clinical models of acute and community services. The Nursing Home Liaison and Support Assistant Director of Nursing will be key to ensuring proactive engagement with the Private sector Nursing Homes in our catchment area, especially during winter planning periods, and with HSE Community Health Organisation Nursing Home support/Intervention team.   The ‘COVID-19 Nursing Homes Expert Panel Examination of Measures to 2021; Report to the Minister for Health’ (2020) outlined key recommendations; that Nursing homes should be part of a continuous spectrum of care of the older person in the wider healthcare system, with provision of multidisciplinary support, better integration with this sector and community and primary care and Acute Hospitals.     The development of this key post is the latest development in the RSCI Hospital Group strategy to ensure; Enhanced Patient and population centred integration across Health services, and specifically our commitment to a comprehensive and integrated Care model for older people consistent with The Integrated Programme for Older People, Sláintecare,and the Covid19 Nursing Homes Expert Panel report to the Minister for Health (2020).   The post holder will have a close working relationship with the Hospital Director of Nursing, the Senior Management Team, the senior nurse management team, the CHO Nursing Home support/intervention team, Directors of Nursing in Nursing Homes in our catchment area, Directors of Nursing and MDT team across Older persons services in Our Hospitals, General Practitioners and primary and community care teams, Palliative Care team. This will be key to ensuring our service offers a safe timely and seamless transition of care between Private Nursing Home and acute and community care consistent with the principle of Right Person, Right Place Right Time.   The post holder will be involved in the management of a 32 bed acute medical ward encompassing care of the older person acute care.   The Cottage Community hub is a 14 bed step down/transitional care unit. It also includes Care of Older Person outpatient assessment clinics run by RANP and Consultant Geriatrician. In addition there is a satellite Outpatient assessment centre in Louth County Hospital   Frailty at the front door team in the Emergency department is presently being fully developed while awaiting appointment of a consultant

The Outreach Team is responsible for improving communication both in and out of the hospital to/from all residential facilities in Cavan/Monaghan to improve the patient’s journey. The Speech and Language Therapy department provides a service to all patients presenting with communication and feeding, eating, drinking and swallowing (FEDS) difficulties in the acute setting. A videofluoroscopy service is also provided.   The aim of the Acute Stroke SLT post is to provide evidence based, high quality speech and language therapy service to patients within the medicine for the elderly service and neuro-rehabilitation service. The post holder will be responsible for the provision of a high quality Speech and Language therapy service in accordance with standards of professional practice. As a Practice Tutor the therapist will develop the capacity and quality of the practice education programme while supporting students and practice educators.   They will carry out the clinical and educational duties as required of a Senior Speech and Language Therapist. CHB - providing a service to acute inpatient areas as the service requires at the Speech and Language Therapy manager’s discretion. NOHC - contribute to the overall operation and development of the communication and swallowing service within the Rehabilitation Service at the National Orthopaedic Hospital, Cappagh in collaboration with the Therapy Services Manager. The candidate will also provide, where requested,  SLT services on the elective orthopaedic service at NOHC. In CHB the Successful candidate will be part of the Stroke/Older Person Rehabilitation Service of Connolly Hospital.  This service encompasses Woodlands Acute Frailty Unit, Rowan Ward (Stroke & Older Person Rehabilitation), Silverbirch Ward (Stroke & Older Person Rehabilitation) & Sycamore Ward (Care of Older Person) and other acute wards.  Flexibility is required as the post holder may be required to work on/between any of the Stroke/Older Person Rehabilitation services.   Professional reporting relationship for clinical governance and clinical supervision will be to the Head of Discipline through the professional line management structure. CHB - This post reports to the Speech and Language Therapy Manager. Supervision may be provided by the Speech and Language Therapy Manager within the department. NOHC - The position reports to and is accountable to the Therapy Services Manager in all matters relating to the job and in their absence reports to the Chief Executive Officer. Clinical supervision relating to the role will be provided by the SLT Manager at CHB.

The overall purpose of the cANP (Respiratory) is to provide safe, timely, evidenced based nurse led care to patients at an advanced nursing level. This involves undertaking and documenting a complete episode of patient care (assess, diagnose, plan, treat and discharge patients) according to collaboratively agreed protocols and scope of practice in the clinical setting, demonstrating advanced clinical and theoretical knowledge, critical thinking and decision making skills.   The cANP in (Respiratory) will play a central role in both the acute/inpatient and outpatient care of patients with respiratory disease. In addition to providing a respiratory nurse service, the respiratory nurses will also assist Connolly Hospital to meet national standards and KPIs, and thereby contribute significantly to reducing hospital admissions, patient length of stay and improving quality of care for patients admitted with respiratory diseases such as COPD and asthma.   The cANP role will extend across the wider Respiratory Service to include:   A. Outpatient Care Facilitate an appropriate level of ANP-led respiratory clinics which are required to facilitate the current and projected demand for respiratory outpatient care within Connolly Hospital.   Specialist Interstitial Lung Disease (ILD) Nurse Care Provide additional respiratory ANP support to the ILD Nurse Service which is rapidly growing in Connolly Hospital, and which is a national and international standard of ILD care.   Supplemental Oxygen (O2) Assist Connolly Hospital to meet KPIs in supplemental O2 therapy e.g. reviewing and initiating referred patients for supplemental O2 therapy as per national guidelines.   B. Acute/Inpatient Care - Support an increased level of respiratory ANP care into acute (e.g. emergency department) and inpatient (e.g. respiratory nurse consult service) respiratory admissions. This will further improve the quality of respiratory care across the hospital, and will further reduce emergency department waiting time, length of stay and admissions. - Assist Connolly Hospital to meet national KPIs in COPD care that have been shown to reduce the number of inpatient admissions and improve length of stay, such as early iv/oral antibiotic switch and discharge plans. - Provide leadership on evidence-based initiatives to develop the quality and efficiency of respiratory care within Connolly Hospital. - Support the development of an ANP-led non-invasive ventilation inpatient service and follow-up clinic which is required to evaluate and monitor patients on this treatment. This campaign will run in compliance with the Code of Practice prepared by the Commission for Public Service Appointments (CPSA). The CPSA is responsible for establishing the principles that should be followed when making an appointment. These are set out in the CPSA Code of Practice. The ADON Emergency Dept / Patient Flow has a key role in the: - Management of patient flow to and from the ED, Assessment units & medical Short Stay Unit to other departments within the Hospital and externally. - Engage and influence the performance of the Hospital patient flow pathways and support systems to facilitate optimum efficiency and effectiveness and safe, responsive high quality patient services. - Provide leadership and be accountable and responsible to further develop governance structures and within those have the authority to assess, plan, action, improve and review unscheduled care patient flow. - In order to enhance the effectiveness of patient flow across the Hospital, engage and influence the prioritisation of patient access to diagnostic facilities in collaboration with clinicians and other key flow management personnel. - Work and collaborate with colleagues, who have responsibility to drive processes essential to timely discharge such as Plan for every patient, discharge planning, predicted date of discharge; criteria led discharge, provision and use of area that support discharge/transit etc. - Develop and enhance relationships across the Primary and Community Care structures creating a collaborative culture in order to improve integrated working and problem solving to reduce unnecessary attendances and support early discharge to the most appropriate setting. - Collaborate with established clinical programmes to facilitate timely patient flow and on the introduction of new pathways, (clinical and organisational) and will monitor, using data, analysis and intelligence to determine effectiveness and report on the performance of the patient pathways to the unscheduled care governance group and the executive management team. - Identify and support opportunities to develop IT practices to enhance patient care and flow, and to increase efficiency while decreasing workload e.g. the development of systems to facilitate rapid ordering, tracing, and review of diagnostic testing, electronic bed mapping and navigational hub. - Ensure that relevant timely data is available to support decision making and to assess quality improvement deliverables. - Involvement in ongoing workforce planning to ensure the availability of appropriate numbers, and levels, of staff to meet patient need. Including identifying opportunities for the development and roll out of advanced nursing practice roles which will increase the quality of patient care and clinical effectiveness, assist with departmental efficiency, and improve patient flow. - Lead on the use of Quality improvement methodologies to ensure the delivery of quality improvement projects that will provide safer better healthcare across the Unscheduled Care patient pathway. - Lead on the Quality Assurance and Verification agenda to ensure the delivery of safer better healthcare across the Unscheduled Care Patient Pathway. - While recognising that the escalation policy contains automatic trigger, the ADON will be responsible for ensuring that the policy is promptly implemented and recorded as implemented in accordance with the provisions of the escalation policy, and to ensure that the Escalation policy overall, is subject to regular and rigorous review. - Ensure as part of the management team that prompt action is taken to identify general & specific health & safety risks.
