The advanced practice service is provided by nurses who practice at a higher level of capability as independent, autonomous and expert advanced practitioners. The overall purpose of the service is to provide safe, timely, evidenced based nurse-led care to patients at an advanced nursing level. This involves undertaking and documenting complete episodes of patient care, which includes comprehensively assessing, diagnosing, planning, treating and discharging patients in accordance with collaboratively agreed local policies, procedures, protocols and guidelines and/or service level agreements / memoranda of understanding.   The RANP (Respiratory) demonstrates advanced clinical and theoretical knowledge, critical thinking, clinical leadership and complex decision-making abilities.   The RANP (Respiratory) practices in accordance with the Code of Professional Conduct and Ethics for Registered Nurses and Registered Midwives (NMBI, 2014), the Scope of Nursing and Midwifery Practice Framework (NMBI, 2015), Advanced Practice (Nursing) Standards and Requirements (NMBI, 2017), and the Values for Nurses and Midwives in Ireland (Department of Health, 2016).   The RANP (Respiratory) provides clinical leadership and professional scholarship in the delivery of optimal nursing services and informs the development of evidence based health policy at local, regional and national levels.   The RANP (Respiratory) contributes to nursing research that shapes and advances nursing practice, education and health care policy at local, national and international levels.   The RANP (Respiratory) will complement and contribute to the overall service provided by the Respiratory department. For all informal enquiries please contact recruitment@rcsihospitals.ie   As outlined in this job description, the cCNS pathway will facilitate the post holder to be supported to professionally and clinically develop the skills and knowledge required to achieve the competencies of the CNS role.   The cCNS post holder will be enabled to deliver care in line with the five core concepts of the role set out in the Framework for the Establishment of Clinical Nurse/Midwife Specialist Posts, 4th edition, National Council for the Professional Development of Nursing and Midwifery (NCNM) 2008.     Caseload The cCNS is responsible for the safe and effective care of patients with diabetes. This involves the direct liaison with the patient and family in the provision of education and assist in their safe management. This includes frequent telephone contacts, continuous education with regard to insulin adjustment and illness management, detection and prevention of hypoglycaemia and diabetic ketoacidosis. The aim is to provide a sound knowledge base at the time of diagnosis, based on a thorough need’s assessment and to build on this thereafter, providing the support and education to enable the child and family to make informed healthcare decisions.     The cCNS clinical role is based on the core concepts and associated competencies for the CNS (adapted from NCNM 4th edition 2008) The concepts are: - Clinical Focus (Direct and Indirect Care) - Service user/Service User Advocacy - Education and Training - Audit and Research - Consultancy (including leadership in clinical practice)   Background to the cCNS Pathway   The Policy on the Development of Graduate to Advanced Nursing and Midwifery Practice (DOH, 2019) outlines a pathway to support a change for specialist practice to develop and meet service need, including the revised eligibility criteria for CNS as follows:   - A minimum of 1 years’ experience in practice prior to completing the required post graduate qualification in the specialist area at level 9 (equivalent to 60 ECTS or above) - Recognition as a CNS to meet service needs therefore requires the completion of a postgraduate qualification together with a minimum of 1 years’ specialist experience - This is in addition to meeting the requirements set by the HSE to ensure the delivery of safe, effective practice to meet service demands   In response to an identified service need a candidate CNS pathway has been established and sets out the following:     - Nurses/Midwives currently employed as Staff Nurses (SN) and Enhanced Nurses (EN) and any other nursing grades who have a minimum of 1 year’s clinical experience and who express an interest in CNS roles could be supported to progress on a candidate CNS pathway as follows:   - A recruitment campaign will be organised by the CHO/HG between local HR and services. - Nurses/midwives who are successful at interview for candidate CNS post will: - be offered a contract for a candidate CNS post (Paediatric Diabetes) at CNM1 grade with a job description outlining the cCNS competencies to be achieved. - be required to demonstrate their specialist experience or if they do not possess the relevant specialist experience, they will be supported to attain one year’s clinical specialist experience - be required to demonstrate that they have completed the required postgraduate education, or agree and are supported to undertake the required postgraduate education, at level 9 qualification (equivalent to 60 ECTS or above) relevant to the specialist area. - be required to demonstrate that they have continuing professional development (CPD) relevant to the specialist area or will be supported to obtain the required CPD. - All of the above must be achieved within 2 years from appointment for this pathway.   - For Nurses/Midwives who express an interest in CNS roles and who currently hold a level 8 educational qualification in the specialist area (equivalent to 60 ECTS or above), this qualification will be recognised up to September 2026. The clinical experience requirements for this cohort of nurses/midwives remain consistent with the DoH (2019) policy, i.e. a minimum of 1 years’ experience in practice and a minimum of 1 years’ experience in the specialist area and they could be supported to progress on a candidate CNS Pathway.   The above pathway is valid from May 2023 to September 2026   Duration of the post:   The candidate CNS is required to progress to Clinical Nurse Specialist grade within 2 years of commencement of this pathway and formally apply for entry onto the Interim ONMSD CNS database (until the database is transferred to its permanent location)

For all informal enquiries please contact recruitment@rcsihospitals.ie   The additional post holder will manage the nursing team in the Emergency Department including Paediatrics, Acute Medical Assessment Unit, Admission Treatment and Investigation Unit in Our Lady of Lourdes Hospital. Roles and responsibilities will be discussed at job offer stage.   The Emergency Department at Our Lady of Lourdes Hospital, Drogheda provides an independently staffed Adult and Paediatric emergency service for the populations of Louth, Meath, Monaghan and North County Dublin.  The department treated approximately 64500 patients in 2019 with 30% of these paediatric presentations. The department is the trauma centre for the North East and one of the busiest nationally. The department has established Advanced Nurse Practitioner services in Minor Injuries, Rapid Assessment and Treatment and Acute Medicine. We have undertaken a major capital development project, which has seen the expansion of the Emergency Department incorporating resuscitation rooms, treatment areas, three high observations bays, a Clinical Decision Making Unit (CDU) an expanded Paediatric ED and designated Minor Injury Unit...   The Emergency Department division includes the Acute Medical Assessment Unit (AMAU), Admission, Treatment and Investigation Unit (ATIU), the Dundalk Injury Unit as well as both adult and paediatric Emergency Departments.  The department has two clinical facilitator who will provide orientation to the department and facilitate the many continual professional development (CPD) opportunities.     The CNM3 in Emergency Medicine has day to day operational responsibilities which  include the following;   - The promotion of best practice that is evidence based with the ultimate outcome of improving patient care and outcomes. - Provide senior and expert professional advice to the Nursing and wider care team and the Director of Nursing. - Ensure appropriate skill-mix and efficient deployment of staffing and other resources across departments. - Ensure appropriate professional development of staff is in place via PDP and training needs assessment. - The implementation of the Quality Safety and Risk Framework within associated specialities - The continuous development of service based clinical governance teams. - The development and monitoring of key Service Objectives in line with local and national service and professional requirements. - The delivery of the National, Louth Meath Hospital Group service plan. - Maintaining a collaborative approach to multidisciplinary and inter departmental working. - Recruitment, retention and monitoring of staff performance within the service. - Establishing and promoting effective channels of communication internally within the department and with other departments in order to provide a seamless service. - The operational management of the nursing teams in the Emergency Medicine Division including Human Resources and Finance functions to support effective and efficient use of resources. - Providing on-going practical and psychological support for nursing staff and other members of the multidisciplinary teams. - The post holder will support the implementation of the group Nursing Strategy.   This role will develop in line with service needs as defined by the Director of Nursing and on-going service developments within the Hospital group.

The Patient Flow Manager will provide Professional and Clinical leadership within the patient flow team and throughout Connolly Hospital.    The focus of the role is to improve the patient experience and quality of care throughout the hospital throughout the hospital while demonstrating dignity, compassion and respect.    This essence of this role is to work within the Acute setting and as a CNM2 or equivalent HSCP, overseeing the identification and seamless, safe transfer of patients from the Acute setting to our Step-down facilities. The role will involve close cooperation and communication with a wide variety of stakeholders both internally and externally, members of the multi-disciplinary team, patient flow team, community services, long term care facilities and step down facilities.  The post holder will possess the flexibility and knowledge to work within the various aspects of the patient flow department as in delayed discharges, long term care, home care packages and transitional care funding.    This senior role will ensure the management and demand for the service as well as addressing capacity improvements, operational efficiencies and transfer of care pathways.    The post is based in the patient flow Operational Hub.   Informal Enquiries: Denise Hartigan, ADON Patient Flow Denise.Hartigan@hse.ie
