The person appointed to this new permanent, full time; Senior Occupational Therapist position will work as part of the new Early Supported Discharge (ESD) service to support early discharge for Stroke patients.   The aim of the Early Supported Discharge (ESD) service is to support people with stroke to be discharged early from hospital and to rehabilitate them in their home environment. The occupational therapist will have a greater understanding of patient needs in their home environment. They can support the patient with stroke to maximise their functional potential, to achieve their goals, to access their environment and return to previous occupations / leisure pursuits and involvement in their community.   The service will be staffed by a senior occupational therapist, a senior physiotherapist and a senior speech and language therapist. They will work closely with the Clinical Nurse Specialist in Stroke and physicians specialising in Stroke.   The person appointed to this Senior Occupational Therapist post will report to the Occupational Therapist Manager, the Head of Discipline of the service (or her designated person / clinical specialist Occupational Therapist as appropriate, through the department’s professional line management structure).   The Senior Occupational Therapist will be responsible for the provision of a high-quality Occupational Therapy service to the patients with stroke who are selected for Early Supported Discharge.   The Senior Occupational Therapist will develop and maintain high standards of clinical practice within their assigned area through clinical evaluation, audit opportunities and evidence-based practice.   The Senior Occupational Therapist will contribute to a structured process for professional supervision of senior and staff grade colleagues, occupational therapy assistants, undergraduates in occupational therapy and other disciplines in collaboration with the occupational therapist manager and clinical specialist occupational therapists in the department.   Reporting Relationship Your professional reporting relationship for clinical governance will be to the Occupational Therapist Manager in Charge 111, in Connolly Hospital, the Head of Discipline of the service (or her/his designated person as appropriate, through the department’s professional line management structure. The person appointed to this Senior Occupational Therapist post will work as part of the multi-disciplinary team delivering a service to the Frailty Intervention Team (FIT) in Connolly  Hospital. The service is based in the Emergency Department and the successful candidate will be responsible for provision of specialised Occupational Therapy intervention, for the frail elderly population presenting to this service. This full-time position is permanent.   The Senior Occupational Therapist on the Frailty Intervention Team will work closely with the Consultant Geriatrician, Medical Registrar, Advanced Nurse Practitioner, Senior Physiotherapist and administrator for the service.   The service was established in 2022 and we have developed close links with our community counterparts (which funds this initiative) including the Integrated Care Team for Older Persons (ICPOP) in CHO 9. The Outreach Team is responsible for improving communication both in and out of the hospital to/from all residential facilities in Cavan/Monaghan to improve the patient’s journey. The aim of the service is to reduce the readmission and transfer of frail older people through the emergency services by co-ordinating their care with the appropriate services and to facilitate rapid access to specialist care as appropriate.  The post of CNM 2 has a pivotal role in service planning, co-ordinating, and managing activity and resources within the outreach service to residential facilities across the catchment areas of Cavan & Monaghan Hospital. The main responsibilities are: quality assurance, resource management, staff development, practice development, facilitating communication and professional / clinical leadership.   This role will provide liaison support to all residential facilities in the catchment area, with the provision of enhanced nursing assessments, reviews (virtual and face to face) and training to support the resident being managed in the residential care setting.  In developing the service, the CNM 2 will provide nursing expertise, education and a consultation service to patients/staff. The post will be based in the Emergency Department (ED) and AMAU Cavan General Hospital and will support the inpatient medical and surgical wards, linking closely with bed management / patient flow on a daily basis.  The GP Liaison Nurse will be responsible for improving communication both in and out of the hospital to/from all GP practices in Cavan/Monaghan to improve the patient’s journey.  Operationally report to the Head of Beds and the Director of Nursing for Integrated services.   Professionally accountable to the Director of Nursing for Integrated services.  The aim of the service is to co-ordinate patient care with the appropriate services and to facilitate rapid access to specialist care as appropriate.   The post holder will be responsible for the following: - Communicating and acting as a vital link between the ED, AMAU, medical and Surgical wards, General Practitioners (GPs) and Community Care Services. In this respect, they will liaise with Bed Management, Wards, Departments and other Paramedical Services where necessary. The Speech and Language Therapy department provides a service to all patients presenting with communication and feeding, eating, drinking and swallowing (FEDS) difficulties in the acute setting. A videofluoroscopy service is also provided.   The aim of the Neuro-Rehabilitation SLT post is to provide evidence based, high quality speech and language therapy service to patients within the medicine for the elderly service and neuro-rehabilitation service. The post holder will be responsible for the provision of a high quality Speech and Language therapy service in accordance with standards of professional practice.   They will carry out the clinical and educational duties as required of a Senior Speech and Language Therapist. The Senior Speech and Language Therapist will develop and maintain high standards of clinical practice within their assigned area through clinical evaluation, audit opportunities and evidence based practice.   To work with the Speech and Language Therapist Manager and other members of the department in co-ordinating the service to meet the needs of the service user and the objectives of the organisation.   Successful candidate will be part of the Stroke/Older Person Rehabilitation Service of Connolly Hospital.  This service encompasses Woodlands Acute Frailty Unit, Rowan Ward (Stroke & Older Person Rehabilitation), Silverbirch Ward (Stroke & Older Person Rehabilitation) & Sycamore Ward (Care of Older Person) and other acute wards.  Flexibility is required as the post holder may be required to work on/between any of the Stroke/Older Person Rehabilitation services.   Professional reporting relationship for clinical governance and clinical supervision will be to the Head of Discipline through the professional line management structure.

The role of the CNS will differ according to the needs and configuration of established respiratory services at each site. The purpose of this Clinical Nurse Specialist - Respiratory Integrated Care post is to: provide expertise and specialist nursing services to patients with a respiratory condition both in the hospital outpatient settings and in primary care. The post holder will liaise between acute respiratory services and integrated respiratory services in the community along with other agencies to deliver effective evidenced based care.  They will use resources efficiently to achieve the best possible outcomes in keeping with the NCP Programme model of care and HIQA standards.    The person appointed to this post will work in newly formed Respiratory Integrated Care services.  The post holder will work as part of a multidisciplinary team delivering coordinated evidence based care for patients in primary care whilst liaising closely with secondary care. The CNS RIC will deliver nurse-led clinics to provide support to patients and their GPs in creating management plans, assessing inhaler treatments, assisting with diagnosis development and will provide education to patients and staff.  This post is 50% acute based and 50% community based. There will be a strong focus on service integration and team-working. This post will also involve the core elements of the CNS post to include clinical audit and research.   In order to ensure continuity of service to patients the CNS may be required to rotate/cover other parts of the integrated respiratory service such as Pulmonary Rehabilitation and Outreach.  This will also promote service integration and enhance skillset development.  It will be dependent on identified service needs at each site. If deemed appropriate, site rotation should occur within the first 3-6 months of taking up the post. The Clinical Nurse Specialist (RIC) caseload will focus initially on the following patient groups. - COPD - Asthma    Location of post This is an appointment to Cavan and Monaghan Hospital by the Health Service Executive. The commitment for this post will be for 37.5 hours per week to work within the associated community-based ambulatory care hub. This post may be subject to restructuring in the future to facilitate the re-organisation of services in line with new clinical models of acute and community services.
