The role of the CNC CLL has a number of key objectives:  - Improved quality of life for children and their families - Improved co-ordination of services to children with life limiting conditions so that they can be cared for in the home setting as far as possible. - To facilitate education and training for health and social care professionals in collaboration with relevant stakeholders. - To act as an informed resource and link person for children and family carers and for health and social care professionals involved in the care of children with life-limiting conditions - To link with Children’s Primary Care Services, PHNs, Disability Services, Adult Specialist Palliative Care Teams and voluntary organisations. - To increase awareness of children’s palliative care - To plan, implement, deliver and evaluate care for a caseload of children with life-limiting conditions and their families, in collaboration with local healthcare professionals/carers. This role will apply to both acute and community care settings.  Using UK prevalence rates it is now estimated that there are approximately 4,000 children in Ireland living with a life-limiting condition.   Palliative care for children with Life Limiting Conditions (LLC) has been defined as ‘an active and total approach to care, from the point of diagnosis or recognition, embracing physical, emotional, social and spiritual elements through to death and beyond.  It focuses on enhancement of quality of life for the child/young person and support for the family and includes the management of stressing symptoms, provision of short breaks and care through death and bereavement’’ (Together for Short Lives 2013)   Children’s Palliative Care is therefore provided to a child and family not just in the dying stages but in the weeks, months and years before a child’s death   Children’s Palliative Care in Ireland is underpinned by policy set out in Palliative Care for Children with Life Limiting Conditions in Ireland, A National Policy (2009).   A comprehensive children’s palliative care service needs to function within a co-operative model with close liaison between the general practitioner, community paediatrician, nursing services, children’s hospitals and hospitals with paediatric units.  The post holder will endorse a palliative care approach and promote seamless care between agencies and professionals involved in the child’s care.   The Service will be active in its total approach to care, embracing physical, emotional, social and spiritual elements.  It focuses on enhancement of quality of life for the child and support for the family, and includes the management of distressing symptoms, provision of respite and care through death and bereavement.   The Service will establish close liaison between general practitioner, community paediatrician, disability services, nursing services, children’s hospitals, hospitals with paediatric units and other state and voluntary services as appropriate.   There are currently 12 Clinical Nurse Co-ordinators for Children with Life-limiting Conditions employed by the HSE. Although directly responsible to nursing management within their respective hospitals, the nurses work closely together as part of a national team to develop and implement best practice for children with life-limiting conditions.  This group is currently working on establishing national standardised care practices.   This post - is professionally accountable to the Director of Midwifery (DOM) or designated officer. Will report to the Assistant Director of Nursing or designated officer as required on operational matters.   There is 1WTE post based in Cavan General Hospital, Cavan as part of the RCSI Hospital Group.   Although the post are located in paediatric units and their reporting line is through the respective hospital governance structure the CNC works across the hospital and community setting. Connolly Hospital Blanchardstown is a major teaching hospital providing a range of acute medical and surgical services, day care, outpatient, diagnostic and support services.  Connolly Hospital is located in the West of the city and has ample free parking for hospital staff.  Emergency services are provided on a 365-day, 24 hour basis.  Multi-disciplinary teams representative of medical, nursing, allied health professionals, management and general support staff play a pivotal role in the development, delivery, monitoring and evaluation of these services.    The hospital is affiliated to the Royal College of Surgeons in Ireland (RCSI) for medical education, to Dublin City University (DCU) for nursing education and to University College Dublin (UCD), Trinity College Dublin (TCD) and the Institute of Technology for allied health professional education.   Connolly Hospital is one of 8 hospitals in the RCSI Hospitals Group covering a population of over 800,000 people.  Connolly Hospital’s Catchment Population of over 331,000 covers Dublin West, North Kildare and South Meath.   The HSE offer an attractive and competitive relocation package for international candidates.  The relocation package includes allowances payable to candidates to meet initial costs for relocating such as an accommodation and flight allowance up to €4,160 for EU/UK candidates and €4,710 for Non-EU candidates.  Additional costs such as registration fees to Regulatory Bodies, Visa fess etc. are also payable on behalf of the candidate.   The Physiotherapy Department provides a comprehensive in-patient & outpatient service.  Specialties include Care of the Older Person, Stroke Service (incl. ESD), ED, MSK, Acute Rehabilitation, Orthopaedics, Rheumatology, Respiratory (Medical & Surgical), Cardiac/Pulmonary Rehabilitation & Occupational Health.   There is also a COPD Outreach Service, Integrated Respiratory Care and a Frailty Intervention Team.   An out of hour’s emergency service and week-end service is provided.    To further enhance Service Provision to those living with symptoms of chronic respiratory disease, the post holder will lead an out-patient respiratory physiotherapy service in collaboration with the respiratory clinics and integrated respiratory care service.   The Clinical Specialist Physiotherapist, as part of the Respiratory team will:   - Establish and deliver a dedicated respiratory physiotherapy out-patient service, accepting referrals from respiratory clinics on site. - This post will facilitate specialist interdisciplinary assessment and care required by patients with chronic respiratory disease, this may include on occasion, post-acute Covid patients. - Work as part of a multidisciplinary team delivering coordinated evidence based care for patients with chronic respiratory disease. - Work with colleagues to deliver long term oxygen therapy & ambulatory oxygen clinics. - Work in conjunction with other Physiotherapy team members in delivering Physiotherapy services as part of the acute services team. - Embrace service redesign as appropriate for the respiratory service.   Flexibility is required as cross cover will be required across the wider In-Patient Respiratory team based on service demands.   The Post Holder will carry out clinical duties and educational duties as required, while developing the Physiotherapy service in line with National Clinical Care Programme.    The successful candidate will work with the Physiotherapy Manager, Respiratory Consultants and Nurse Specialist colleagues in ensuring the co-ordination, development and delivery of a quality, client centred service across all relevant clinical spheres.   The Clinical Specialist Physiotherapist will report to the Head of Discipline for clinical governance and clinical supervision through the professional line management structure, and will also be accountable to the Integrated Respiratory Consultant lead. This post requires a RANP / cANP (IBD) with a specific scope of practice for Crohn’s disease, Ulcerative colitis and overlap care across adult age groups (age 16 and upwards).   The main purpose of the post is to develop the job description and supporting documentation under the direction of the Health Care Provider’s Advanced Practice Stakeholder Governance Group, to enable the individual nurse to meet the NMBI Criteria for Registration as an Advanced Nurse Practitioner as set out in Advanced Practice (Nursing) Standards and Requirements (NMBI, 2017).   The individual will undertake the academic preparation and develop the clinical and leadership skills, competencies and knowledge required to meet the criteria to be registered as a RANP with NMBI. The scope of the cANP role must reflect the incremental development of expertise and as such, the cANP cannot deliver care as an autonomous practitioner.   The individual will complete and submit the necessary documentation for registration as an RANP with NMBI.   The value of the nursing contribution as a distinct profession must be safeguarded and articulated in the development of new services led by advanced nurse practitioners, complementing rather than replacing current services delivered by doctors (NMBI 2017 p.9).   The overall purpose of the post is to provide safe, timely, evidenced based nurse-led care to patients at an advanced nursing level. This involves undertaking and documenting a complete episode of patient care (assess, diagnose, plan, treat and discharge patients) according to collaboratively agreed protocols and scope of practice in the clinical setting; demonstrating advanced clinical and theoretical knowledge, critical thinking, clinical leadership and decision making skills.   The advanced practice role demonstrates a high degree of knowledge, skill and experience that is applied within the nurse-patient/client relationship to achieve optimal outcomes through critical analysis, problem solving and accurate decision-making (NMBI, 2017). Central to this is the provision of quality care, a safe environment and processes for patients by the use of evidence based clinical guidelines that address patient expectations, promote wellness and evaluate care given.   The role will provide clinical leadership and professional scholarship in order to develop nursing practice and health policy at local, regional and national level.   The role will contribute to nursing research to shape and advance nursing practice, education and health care policy at local, national and international levels.   The RANP / cANP (IBD) has a role in the ongoing development of the service within CMH (age 16 and upwards).   Once the cANP (IBD) meets the registration requirements of cANP (IBD) following completion and submission of the necessary documentation with NMBI, the cANP (IBD will) complement and contribute to the overall service.   The specific contribution of the RANP / cANP (IBD) is to improve outcomes for patients who fall within the RANP / cANP scope of practice and improve access to timely, quality healthcare for patients with IBD disease.   The inclusion criteria for the RANP / cANP (Inflammatory Bowel Disease) are as follows:  - Patients over the age of 16 years with a confirmed diagnosis of IBD who have been referred to Adult services in Cavan Monaghan Hospital. - All IBD patients who require an assessment or a review assessment.  *The caseload and scope of practice for the registered advanced nurse practitioner services will evolve to reflect changing service needs.   The exclusion criteria for the RANP / cANP (Inflammatory Bowel Disease) are as follows:  - Patients under the age of 16 years - Patients who do not have a confirmed diagnosis IBD  Specific requirements to support the development of a RANP / cANP IBD service.  - Clinical assessment including health history, interpretation of signs and symptoms, interpretation of diagnostic tests, physical examination, differential diagnosis and diagnosis according to the outcomes of the assessment. - Knowledge of pharmacological and non-pharmacological therapies. - Knowledge of psychosocial interventions. - Care for patients across a range of care settings requiring communicating/collaborating with relevant key stakeholders. - Knowledge of quality improvement processes and principles. - Knowledge of health promotion principles of Registered Nurse Prescriber. - Involvement with Academic Third Level institutions regarding education, research, future IBD care developments etc. - Involvement in multidisciplinary education. - Conduct Research in clinical setting and conduct Audits on all aspects of the service including experience of service users and health care professionals. - Involvement with Academic Third Level institutions regarding education, research, future IBD care developments etc. - Registered Ionising Radiation Prescriber. The Physiotherapy Department provides a comprehensive in-patient & outpatient service.  Specialties include Care of the Older Person, Stroke Service, ED, MSK, Acute Rehabilitation, Orthopaedics, Rheumatology, Respiratory (Medical & Surgical), Cardiac/Pulmonary Rehabilitation & Occupational Health.   There is also a COPD Outreach Service, Integrated Respiratory Care and a Frailty Intervention Team.   Staff Grade rotations are four months duration.  There is a strong ethos of professional development within the Physiotherapy Department with dedicated time for continuous professional development (CPD) across all services.  All Staff Grades are required to complete a project during their rotation and present same at the end of each rotation.  The Physiotherapy Department has strong links with both Royal College of Surgeons in Ireland (RCSI) & University College Dublin (UCD); providing student placements for undergraduate programmes, while affording Staff Grade Physiotherapists the opportunity to supervise undergraduates on clinical placement.   An out of hour’s emergency service and week-end service is provided.      Professional reporting relationship for clinical governance and clinical supervision will be to the Head of Discipline through the professional line management structure.   This campaign will run in compliance with the Code of Practice prepared by the Commission for Public Service Appointments (CPSA).
