Overview

The post holder will work collaboratively with the ANP (Advanced Nurse Practitioner) Dementia Care to promote integrated working across acute and community services sites.

 

The CNS Dementia Care will:

  • Have an operational clinical working relationship with the Geriatrician(s) leading on Dementia Services within CHB
  • Be required to work as a key member of the relevant specialist team.
  • They will provide comprehensive physical, psychological and emotional assessment and support to people with dementia and their families/carers throughout their disease trajectory.
  • Use a case management approach, they will act as a liaison between Services for Older People and other relevant departments Primary Care, Community services and other agencies in order to provide an integrated service for people with dementia and their carers/families.

The appointment of the CNS Dementia Care is an essential element in the provision of effective, high quality, seamless integrated care for people with dementia.

 

The CNS post holder will deliver care in line with the five core concepts of the role set out in the Framework for the Establishment of Clinical Nurse/Midwife Specialist Posts, 4th edition, National Council for the Professional Development of Nursing and Midwifery (NCNM) 2008.

  • Clinical Focus (Direct and Indirect Care)
  • Service user/client Advocacy
  • Education and Training
  • Audit and Research
  • Consultancy (including leadership in clinical practice)
  •

Essential Criteria

Candidates must, at the latest date for receipt of completed applications for the post:

 

Statutory Registration, Professional Qualifications, Experience, etc

Be a registered nurse on the active Register of Nurses and Midwives held by An Bord Altranais agus Cnáimhseachais na hÉireann (Nursing and Midwifery Board of Ireland) or be eligible to be so registered.

 And

 

Be registered in the General Division of the register of Nurses and Midwives in which the application is being made.

Or

 

In exceptional circumstances, which will be assessed on a case by case basis be registered in another Division of the register of Nurses and Midwives.

And

 

Have a minimum of 1 years’ post registration full time experience or an aggregate of 1 years’ full time experience in the division of the register in which the application is being made

And

 

Have a minimum of 1 years’ full time experience or an aggregate of 1 years full time experience in specialist area of Dementia care.

And

 

Have successfully completed a post registration programme of study, as certified by the education provider, which verifies that the applicant has achieved a Quality and Qualifications Ireland (QQI), National Framework of Qualifications (NFQ) major academic Level 8 or higher relevant to the specialist area of Dementia Care prior to application

Or

 

Can provide written evidence from higher education institute that they have achieved the number of ECTS credits equivalent to a Level 8 or higher standard, relevant to the specialist area of Dementia care nursing prior to application (See **Note 1 below).

 And

Demonstrate evidence of continuing professional development.

 

And

 

Have the ability to practice safely and effectively fulfilling his/her professional responsibility within his/her scope of practice

 

 

Post Specific Requirements: 

  • Demonstrate depth & breadth of post registration nursing experience in the specialist area of Dementia Nursing as relevant to the role
  • Current experience working in the area of Gerontology
