Overview

To ensure;

  • Effective discharge and transfer of patients from Our Lady of Lourdes Hospital, Louth County Hospital, the Cottage Community Hub or any external residential bed capacity available to the Hospitals to their place of residence / community or other appropriate setting utilising a multi-disciplinary approach.
  • Nursing and other professional staffs are empowered to establish effective and appropriate discharge arrangements for in-patients.
  • An integrated person centred approach is adopted, which considers and responds to the holistic needs of individuals admitted to and discharged from Our Lady of Lourdes Hospital, Louth County Hospital, the Cottage Community Hub or any other residential external bed capacity
  • Processes are in place to support the smooth and timely transition of individuals from hospital to alternative settings appropriate to their needs and preferences that support the continuity of care, as appropriate.

Organisational Context

The HSE has created six new health regions. Each region is responsible for providing both hospital and community care for the people in that area. Bringing community health services and hospitals together means we can take a more patient-centred approach to healthcare.

 

HSE Dublin and North East provides health and social care to North Dublin, Louth, Meath,

Monaghan and most areas of Cavan.

 

HSE Dublin and North East Region includes the following hospitals;

  • Beaumont Hospital
  • Cavan General Hospital
  • Connolly Hospital
  • Louth County Hospital
  • National Orthopaedic Hospital Cappagh
  • Monaghan General Hospital
  • Mater Misericordiae University Hospital
  • Our Ladys Hospital Navan
  • Our Lady of Lourdes Hospital
  • Rotunda Hospital

Essential Criteria

Each candidate must, at the latest date for receipt of completed applications for the post:

 

  1. Professional Qualifications & Experience

 

  • Be registered in the General Division of the Register of Nurses maintained by Bord Altranais agus Cnáimhseachais na hÉireann (Nursing and Midwifery Board of Ireland) or be entitled to be so registered

 

And

 

  • Have at least 5 years post registration experience of which 3 must be in an acute setting.

 

And

 

  • Have the clinical, managerial and administrative capacity to properly discharge the functions of the role

And

 

  • Demonstrate evidence of continuing professional development at the appropriate level

 

 

  1. Annual registration
  2. Practitioners must maintain live annual registration on the relevant division of the Register of Nurses and Midwifes maintained by the Nursing and Midwifery Board of Ireland (An Bord Altranais agus Cnáimhseachais na hÉireann)

 

  1. Confirm annual registration with NMBI to the HSE by way of the annual Patient Safety Assurance Certificate (PSAC).
