Details of Service

The Cavan Monaghan Diabetes Integrated Service is dedicated to providing services based on evidence based best practice.  The RANP Diabetes Integrated Care is primarily focused on promoting new ways of working collaboratively to improve the quality of care delivered by those who work within the whole care system (DOH 2019). 

 

The RANP (Diabetes Integrated Care) works within Chronic Disease Hub/Primary Care setting in Cavan/Monaghan.  The RANP (Diabetes Integrated Care) practices in collaboration with the Consultant (Diabetes Integrated Care) and Diabetes Integrated Multidisciplinary Team.

The objective is to promote safe, quality, person-centred care delivery for patients who are included within the RANP agreed caseload, providing timely and safe access to care.  The RANP works in collaboration with the Diabetes team within the geographical catchment area of Cavan Monaghan, supporting an integrated approach to the provision of care to patients with Diabetes conditions, through the Integrated Model of Care for people with Type 2 Diabetes Mellitus (2024).

 

Location

A panel may be created for Registered Advanced Nurse Practitioner in Diabetes Integrated Care from which permanent and specified purpose vacancies of full or part time duration may be filled in CH Cavan/Monaghan, Cavan/Monaghan IHA.

Essential Criteria

Eligibility Criteria Qualifications and/ or experience

 

 

Eligible applicants will be those who on the closing date for the competition:

 

 (i) Are registered in the Advanced Nurse Practitioner division of the Nursing

and Midwifery Board of Ireland Register maintained by the Nursing & Midwifery Board of Ireland [NMBI] (Bord Altranais agus Cnáimhseachais na hÉireann).

                                                          Or

 (ii) Be eligible to register in the Advanced Nurse Practitioner division of the

Nursing and Midwifery Board of Ireland Register maintained by the

Nursing & Midwifery Board of Ireland [NMBI] (Bord Altranais agus

Cnáimhseachais na hÉireann) by meeting the criteria for registration as

an Advanced Nurse Practitioner as specified www.nmbi.ie

                                                        And

 (iii) Have a broad base of clinical experience relevant to the advanced field of

Practice (Diabetes)

                                                        And

 (iv) Demonstrates the competences relevant to the specialist area of advanced

Practice (Diabetes)

                                                 And

(b) Candidates must possess the requisite knowledge and ability including a high

standard of suitability and clinical, professional and administrative capacity to

properly discharge the functions of the role.

 

Annual registration

 (i) On appointment, practitioners must maintain live annual registration on the Advanced Nurse Practitioner Division of the register of Nurses and Midwives maintained by the Nursing and Midwifery Board of Ireland (Bord Altranais agus Cnáimhseachais na hÉireann) for the role

 

                                                        AND

 (ii) Practitioners must confirm annual registration with NMBI to the HSE by way of the annual Patient Safety Assurance Certificate (PSAC).

 

Health

 Candidates for and any person holding the office must be fully competent and capable of

undertaking the duties attached to the office and be in a state of health such as would

indicate a reasonable prospect of ability to render regular and efficient service.

 

 Character

 Candidates for and any person holding the office must be of good character.

 

Post Specific Requirements

Demonstrate depth and breadth of nursing experience in the specialist area of Registered Advance Nurse Practitioner (Diabetes) as relevant to the role.

Other requirements specific to the post: Full drivers licence and Access to appropriate transport to fulfil the requirements of the role

 

Skills & Competencies

Skills, competencies and/or knowledge

The RANP (Diabetes) will be required to continue to demonstrate the ability to practice at a higher level of capability across six domains of competence as defined by (NMBI 2017), along with the specialist knowledge and clinical skills in the (Diabetes) area of practice.

The RANP (Diabetes) must continue to demonstrate:
