The Clinical Nurse Specialist (Community Mental Health) has a pivotal role in the delivery of effective, recovery orientated, high quality, team based, integrated care for individuals with mental health problems. The purpose of the role is also to provide assessment and support to the individual through the delivery of appropriate treatment and recovery orientated interventions to support care in the community working towards reduction in the need for acute hospitalisation. This will include: domiciliary visits, family support and liaison with other services. They will involve the staff members undertaking assessments, initiating a range of evidenced based recovery orientated clinical interventions and linking closely with a range of statutory services in secondary and primary care services and with voluntary services in the local community. They will be expected to work as a member of a community mental health team, undertaking standardised assessments and management along with assertive follow up involving families/carers as appropriate.
The purpose of this Clinical Nurse Specialist (Community Mental Health) post is to: Deliver care in line with the five core concepts of the role set out in the Framework for the Establishment of Clinical Nurse/Midwife Specialist Posts, 4th Edition, National Council for the Professional Development of Nursing and Midwifery (NCNM) 2008.
Caseload: The caseload of the Clinical Nurse Specialist (Community Mental Health) will be service users attending the identified community mental health team or service.
The Clinical Nurse Specialist (Community Mental Health) will operate within their own scope of practice.
