Overview

The GP Liaison Nurse (CNM2) will lead the delivery of an integrated model of communication between hospital services and General Practitioners (GPs) across Cavan and Monaghan. A key function of this role is to enhance the quality and timeliness of discharge information, ensure effective transitions of care, and support GP practices in managing patients safely and efficiently in the community.

The post holder will serve as a clinical bridge between acute, emergency, and community settings, with a particular focus on improving the content, consistency, and delivery of GP discharge letters and ensuring GPs are kept informed of critical updates related to their patients.

Essential Criteria

1.  Professional Qualifications, Experience, etc.

(a) Eligible applicants will be those who on the closing date for the competition:

 

(i) Be registered in the General Division of the Register of Nurses & Midwives maintained by Bord Altranais agus Cnáimhseachais na hÉireann (Nursing and Midwifery Board of Ireland) or be entitled to be so registered 

 

And

(ii) Have at least 5 years post registration experience with 2 years’ experience across the acute floor.

 

And

(iii) Candidates must demonstrate evidence of continuous professional development at the appropriate level.

 

And 

(b) Have the clinical, managerial and administrative capacity to properly discharge the functions of the role including  knowledge and understanding of Primary Care and GP services in Cavan and Monaghan

 

2.  Annual registration

 

(i) Practitioners must maintain live annual registration on the relevant division of the Register of Nurses and Midwives maintained by the Nursing and Midwifery Board of Ireland (Bord Altranais agus Cnáimhseachais na hÉireann).

 

And

(ii) Confirm annual registration with NMBI to the HSE by way of the annual Patient Safety Assurance Certificate (PSAC).

 

4.  Health

Candidates for and any person holding the office must be fully competent and capable of undertaking the duties attached to the office and be in a state of health such as would indicate a reasonable prospect of ability to render regular and efficient service.

 

5  Character

Candidates for and any person holding the office must be of good character.

 

Post Specific Requirements

Demonstrate depth and breadth of experience in Discharge Planning / Admission and ED / Acute Floor environment as relevant to the role.

 

Desirable Qualities and Skills

  • Strong interpersonal and communication skills, with the ability to influence and build consensus across sectors.
  • Proven ability to lead service improvement initiatives and manage competing priorities in a complex healthcare environment.
  • Understanding of GP practice operations, hospital discharge planning, and community service interfaces.
  • Commitment to delivering person-centred, safe, and integrated care. 

Other Requirements Specific to the Post

Candidates must have a full drivers licence as regular travel to GP Surgery’s within the catchment area of Cavan and Monaghan Hospital may be required.
