Temporary Consultant Physician in Geriatric Medicine, Connolly Hopsital/ St Mary's
The Catchment Area extends into West Dublin, Meath and Kildare, where much of the recent housing development has occurred to support the rapid growth of Greater Dublin. As a result, the population within its service area has experienced considerable change in the inter-censal period and indeed over the past fifteen years. The CSO Census data from the 1991, 1996, 2002 Census as well as the Preliminary release data from Census 2006 can all be used to track the regions changing population. The catchment includes Areas belonging to Fingal, Dublin City, South County Dublin, Kildare and Meath County Councils.
The Geriatric Medicine service in Connolly Hospital covers acute, rehabilitation, stroke and ambulatory care services for older people.
The Geriatric Medicine Community Liaison Service that this post covers works across acute hospital and the local area in CHO 9 including a remit to St Mary’s Hospital, Phoenix Park and private and public nursing homes in the catchment over a split sessional basis.

Summary of Post:
The post holder will be the clinical lead for the community liaison service with a specific remit to support existing care pathways in the community and acute hospital for frail older people. The intention is to ensure that frail older people in community settings can access supported age-attuned pathways which provide alternatives to acute hospital admission and in the case of residents admitted from nursing homes can avail of early supported discharge where appropriate. These include lead clinical governance for patients admitted from long-term care settings for older people, outreach activity to support nursing home residents in long-term care, clinical governance for integrated care dementia pathway, ambulatory care pathway in day hospital for older persons and community response beds, Elms Ward St Mary’s Hospital. A network of clinical teams has been developed for these services to support these pathways including advance nurse practice roles, clinical nurse managers, case managers and multidisciplinary teams. The consultant geriatrician role provides lead clinical governance (with colleagues) for these teams within the prescribed caseload.
Standard Duties and responsibilities
• To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan and in line with policies as specified by the Employer.
• To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.
• To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.
• To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.
• To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.
• To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.
• To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.
• To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery.
• To attend at Connolly Hospital at such times as may be determined by Connolly Hospital, and in emergencies as required, and to remain in attendance thereafter as long as his/her services are required..
• To participate in multidisciplinary care and meetings relevant to the care of patients in Connolly Hospital.
• To participate in research and in clinical trials as appropriate in Connolly Hospital and to ensure compliance with ethical and data protection requirements
• Participate in teaching, including undergraduate and postgraduate teaching and conference commitments of the department and of the hospital for medical, nursing and other staff.
• To ensure that duties and functions are undertaken in a manner that minimizes delays for patients and possible disruption of service.
• To work within the framework of the hospitals’ service plans and levels of service (volume, types etc) as determined by Connolly Hospital. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.
• Participate in the development of agreed care protocols.
• To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.
• To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery.
• The Consultant shall, as part of his/her standard contractual commitment, contribute to the education, training and supervision of students, non-Consultant Hospital Doctors and trainee professionals including members of the multi-disciplinary team.
• The Consultant shall, as part of his/her standard contractual commitment, contribute to the advancement of knowledge by facilitating and supporting research.

Entry to competition / recruitment process
No candidate will be appointed as a Medical Consultant unless (s)he is registered as a Specialist in the Specialist Division of the Register of Medical Practitioners maintained by the Medical Council of Ireland.

Registration as a specialist in the Specialist Division of the Register of Medical Practitioners maintained by the Medical Council in Ireland in the speciality of medicine.
Age
Age restriction shall only apply to a candidate where he/she is not classified as a new entrant (within the meaning of the Public Service Superannuation (Miscellaneous Provisions) Act, 2004). A candidate who is not classified as a new entrant must be under 65 years of age on the first day of the month in which the latest date for receiving completed application forms for the office occurs.

A candidate for and any person holding the post must be fully competent and capable of undertaking the duties attached to the post and be in a state of health such as would indicate a reasonable prospect of ability to render regular and efficient service.
