He/She will be responsible for the management of the off-site/stepdown facilities in line with our Winter planning 2020/21.

  • To oversee the management of all admissions, transfers and discharges in Connolly Hospital within a framework that integrates all processes related to unscheduled/scheduled care.
  • Discharge planning is the main focus of the patient flow department which is fundamental to creating capacity across the hospital in order to manage patient flow efficiently and efficiently.

Key Responsibilities

Operational Management

  • Lead and support the Unscheduled Care department to ensure the efficient and effective day to day running of the department.
  • Ensuring that organisational and national targets are met by developing and delivering effective operational plans linked to the hospital strategy.
  • Develop and implement admission, discharge and transfer protocols, guidance and policies and management of emergency workload and the timely placement of patients in the most appropriate environment in the acute or noon-acute sector.
  • Continuous monitoring and reporting of capacity KPS’s to senior management and circulation on a regular basis.
  • Development and completion of reports for key clinical decision makers on resource utilisation within the acute sector.
  • Ensure effective strategies and pathways are in place for the management of complex discharges.
  • Lead and develop policies and departmental strategies to support efficient discharge planning.
  • Manage the development and operational implementation of new service initiatives.
  • Maximise communication with colleagues in primary care and other hospitals/facilities.
  • Maximising and promoting appropriate bed utilisation.
  • Critical analysis of trends in patient activity  such as length of stay, complex discharge compliance, admissions, transfers and discharges.
  • Analysis, monitoring and forecasting trends of admission and discharges in the acute sector.
  • Actively directing and participating in service planning, implementing and evaluating the overall management of resources in the hospital.l
  • Ensuring best practice by developing action plans and continuously improving the effectiveness of patient flow.
  • Authority to work directly with the Medical and Surgical Departments, Nursing Department, Hospital Management, internal and external agencies.
  • Accountable for effective decision making and action taken where required.
  • Responsible for education and training of staff within Patient Flow.
  • Engage with Senior Management in developing service plans.
  • Creating a culture of accountability, openness to change, respect for the individual and teamwork.
  • Enhance and promote the reputation of the department internally and externally.
  • Lead projects which will progress the quality of services provided to patients.

Performance Management

  • Reporting directly to the Unscheduled Care Manager on department activity.
  • Work collaboratively with the Unscheduled Care Manager and Hospital Departments in leading initiatives, strategic planning, development and redesign of services.

HR Management and Organisational Development

  • Complying with hospital HR policies and procedures in leading and managing staff in Patient Flow.
  • Ensure a positive working environment by ensuring a highly focused, skilled, educated and motivated team.
  • Develop systems for performance appraisal, personal development, individual and organisational development and continued skill mix review.
  • Ensure responsibilities and objectives for direct reports are defined clearly and understood.
  • Regularly review staff complements and skill mix across the department in conjunction with the Manager and Deputy of Unscheduled Care.
  • Responsible for managing sickness and absence in accordance with hospital policy.
  • Act as a point of contact for staff on human resources issues and procedures.
  • Responsible for facilitating staff to continue their education and personal development to support improved organisational performance.
  • Involve staff in open and direct communication to sustain a high level of commitment and flexibility from the Patient Flow department.
  • Promote equal opportunity and adherence to best practice employment and approaches to managing and implementing change.

Quality Assurance and Risk Management

  • Lead and contribute to the development/implementation of best practice service modernisation and improvement plans and strategies.
  • Encouraging a culture of continuous improvement and setting/supporting the achievement.
  • Work with the wider hospital, stakeholders and colleagues to develop clinical governance systems within the department.
  • To ensure effective procedures exist for risk assessment and risk management.
  • Co-ordinate the Patient Flow Departments participation in local risk management initiatives.
  • Co-ordinate the processing of adverse incidents within the department.

Key Performance Indicators

  • The identification and development of Key Performance Indicators (KPIs) which are congruent with the Hospital’s service plan targets.
  • The development of Action Plans to address KPI targets.
  • Driving and promoting a Performance Management culture.
  • The management and delivery of KPI’s as a routine and core business objective.

Essential Criteria


Statutory Registration, Professional Qualifications, Experience etc


Eligible applicants will be those who on the closing date for the competition 


  • Are registered, or be eligible for registration in the General Nurse Division of the Register of Nurses and Midwives maintained by the Nursing & Midwifery Board of Ireland (Bord Altranais agus Cnáimhseachais na hÉireann).


  • Have at least 5 years post registration experience in the division of the register in which the applicant is currently practising.


  • Two Years Clinical Nurse Manager 2 managerial experience or equivlent in acute hospital setting.


  • Have the clinical, managerial and administrative capacity to properly discharge the functions of the role


  •  Demonstrate evidence of continuing professional development at the appropriate level


  • Candidates must possess the requisite clinical, leadership, managerial and administrative knowledge and ability for the proper discharge of the office.

Post Specific

  • Expert knowledge of the operational business of a hospital environment
  • Experience of working with a range of personnel at senior level within an organisation

Annual Registration

Practitioners must maintain live annual registration in the relevant division of the Register of Nurses and Midwives, maintained by the Nursing and Midwifery Board of Ireland [Bord Altranais agus Caimhseachais na hEireann] 




Confirm annual registration with NMBI to the HSE by way of the annual Patient Safety Assurance Certificate [PSAC]



A candidate for and any person holding the office must be fully competent and capable of undertaking the duties attached to the office and be in a state of health such as would indicate a reasonable prospect of ability to render regular and efficient service.

Age restrictions shall only apply to a candidate where he/she is not classified as a new entrant (within the meaning of the Public Service Superannuation Act, 2004).  A candidate who is not classified as a new entrant must be under 65 years of age.


Please note that appointment to and continuation in posts that require statutory registration is dependent upon the post holder maintaining annual registration in the relevant division of the register maintained by Bord Altranais agus Cnáimhseachais na hÉireann (Nursing Midwifery Board Ireland)
